This article will guide you through checking the demographic details of the patient.
1. Once we create the appointment, it is successfully listed on the appointment calendar.
2. When you click on the patient’s name from the appointment calendar, their information will be displayed.
3. The patient’s details include the following details, such as.

C. Plan information

A. Basic information
In the basic information, including the patient name , patient ID, Gender, Date of birth, Membership type, 1st referral doctor, 2nd referral doctor, recall patient, Is decreased, is Staff Patient , Allergy
- Patient name include the name of the specific patient
- 1st referral doctors means a written order from your primary care doctor for you to see a specialist or get certain medical service
- 2nd Referral doctors means a patient who has been sent (referred) for a second opinion or therapy to a specialist or subspecialist, because the patient has a disease or condition that the primary or referring physician cannot, or does not wish to,treat.
- Membership Type includes the three types of plan such as
a. Corporate membership is a “premier” corporate membership that includes all the benefits that a corporate member receives.
b. Retail membership is a premier retail membership that includes all the benefits that a retail member receives.
c. Normal membership means coming under the normal patient charges. - Blood Group There are 4 main blood groups (types of blood) – A, B, AB and O. Your blood group is determined by the genes you inherit from your parents.
- Recall Status In the recall status includes the following points
a. On Going
b. In Active
c. Closed
Here, users can update the recall status and click on the update button.
- If patient is female then in the Basic information include the one more point that Is pregnant.
a. Click on the “ is pregnant” option, then display the pregnancy status screen
b. If yes or no then select the pregnancy status.
c. If yes, then select the Date of Delivery
d. Click on the update option and update the details
e. If not, then click on the close option.
- Is Decreased includes the following points
a. Decreased Status includes the two options: yes or no.
I. If a patient is declared dead, then yes, the status will be updated.
II. If a patient is alive, then no, the status will be updated.
d. Date of decrease includes the patient’s death date.
e. Cause of death includes the reason for the patient’s death.
f. Mother Name includes the patient’s mother’s name.
g. Father Name includes the patient’s father’s name.
h. Body Accepted By means of whomsoever accepts the dead body,
i. Nurse who will be doing the final procedure on the body
j. Mortuary official where they can store the dead body
- In staff, patient includes the staff, relatives or staff itself
- In the allergy includes the two option.
a. Add patient allergy.
b. Update patients’ allergies
- In the allergy section, include the following tabs such as
I) The allergy category includes all types of allergies.
II) in the Allergy tab, put in the specific name of the allergy.
III) In the Description box, users can write a description of the allergy.
IV) in the Severity box , users can update the importance of the allergy.
V) In the Recorded on tab either you can add or update the date
VI) In the Recorded by tab, users can update the name of the person who will be recording the details.
B. Contact.
The contact tab includes the three elements, such as
- Patient Email ID
- Patient Mobile Number
- Patient residential address
C. Plan Information.
In the plan information section includes the
- Membership Type:- in the membership type users can select the membership plan according to the type of members
- Insurance Name:- If the patient has a specific plan, only the insurance name will appear in the column.
- Subscription Date :-The subscription date will be reflected according to the membership plan
- Policy Number :- A policy number is a unique number that an insurance company uses to identify the patient as a policyholder.
D. Action.
In the Action tab users can check the following points
- In the Patient Records users can upload the consent form for the patient
- In the Update Patient you can update the patient’s demographic details.
E. Health Management
In the Health management includes Risk Factors and Action plan
- Risk Factors :- In the risk factor users can collect patient data to identify potential risk factors and address concerns.
- Action Plan :- in the action plan, users can check and update the action plan of the patient.
